This is the most useful thing you can send, and the most likely thing to be wrong. Please include the prompt as it appeared and the answer you typed. Every answer typed is already recorded, so reports like this are how missing answers get found and added.

An answer was scored in the wrong tier

Also useful. Rarity is derived from measurement rather than set by hand, which makes it consistent but not infallible — homonyms and ambiguous names are where it is weakest. Include the prompt, the answer, and what tier you think it should have been.

A bug, or a prompt that is ambiguous

Include the prompt text and what happened. Prompts with more than one reasonable reading are worth reporting: they make the game unfair in a way that is invisible from the inside.
